To cancel the Autoload direct debit for your SmartRider.

Users who have applied for SmartRider Autoload.

To issue a SmartRider for a student attending a non-participating school. This includes all primary school students.

All primary and non-participating secondary school students.

• $5.00 purchase fee for secondary students
• No purchase fee for primary students

Application for Replacement Student SmartRider

To issue replacement SmartRider for a student attending a non-participating school. This includes all primary school students.

All primary and secondary school students who have lost their Student SmartRider card.

• $5.00 replacement fee applies.

Student SmartRider - Faulty Card Report form

To report a faulty SmartRider card.

Primary and secondary school students only.

Participating schools - submit form directly to your school. Your school will submit the form directly to Monitor WA.

Seniors Card Application form

To issue a Seniors SmartRider card.

Holders of WA Seniors Card.

Seniors post to:

Level 2, 140 William Street, Perth WA 6000.
(Entry is from lifts in arcade off Murray St Mall).

Not available (Contact WA Seniors Card Centre).

Application for Tertiary SmartRider (Metropolitan)

Application for a tertiary concession SmartRider for full-time students attending non-participating tertiary institutions in the metropolitan area.
